The study results were presented at a session on metoprolol in heart failure at the 5th World Congress on Heart Failure in Washington, D.C. Dr. Harold Kennedy, Professor of Medicine at Rush, reported the results of a preliminary study of extended release metoprolol succinate tablets (Toprol-XL(R)-Astra USA) in 61 people with NYHA Grade II-IV heart failure. This study was a pilot investigation for MERIT-HF (The Metoprolol Randomized Intervention Trial in Heart Failure), a global mortality study of 3,200 heart failure patients. MERIT-HF is designed to compare the effect of extended release metoprolol tablets plus current standard heart failure treatment against current standard treatment alone. Patients are now being actively recruited for participation in MERIT-HF, which is expected to be completed in 1999. As with most beta blocking agents, Toprol-XL is contra-indicated in sinus bradycardia, heart block greater than first degree, cardiogenic shock and overt cardiac failure. Patients taking Toprol-XL should avoid abrupt cessation of therapy. Following abrupt cessation of therapy with certain beta-blocking agents, exacerbations of angina pectoris and, in some cases, myocardial infarction have occurred. The dosage should be gradually reduced over a one to two week period while monitoring patients. Please see full prescribing information, including the Box Warning, on the enclosed package insert. Astra USA is committed to Toprol-XL and research on B1-blockade. Astra USA, Inc. is a wholly owned U.S. subsidiary of Astra AB, Sodertalje, Sweden. Questions on Toprol-XL, beta-blocker usage and MERIT-HF may be addressed to Lloyd Haskell MD, Senior Director of Clinical Research or Conrad Krebs, US Medical Monitor at 1-800-882-7872.
