| | | ![]() Hydralazine, Isosorbide Dinitrate Benefits Patients With Diastolic Heart Failure, Regardless of Race: Presented at ACC By Bruce Sylvester ATLANTA -- March 17, 2010 -- Treatment with hydralazine and isosorbide dinitrate (HISDN) is associated with improved 1-year survival in both blacks and nonblacks diagnosed with diastolic heart failure, researchers reported at the of the American College of Cardiology 59th Annual Scientific Session (ACC). “To my knowledge, this is the first trial to look at HISDN for diastolic heart failure,” said Jeremy Mazurek, MD, Jacobi Medical Center, and Albert Einstein College of Medicine, Bronx, New York, on March 15. “This treatment has already been shown to improve mortality and symptoms in systolic heart failure, specifically improving mortality in the black population,” he said. “This study shows that the benefit holds true for diastolic heart failure, with an important mortality benefit for nonblacks as well.” The investigators performed a retrospective cohort analysis of patients admitted to and followed at the Montefiore Medical Center, Bronx, New York. Patients were aged >16 years and had a diagnosis of diastolic heart failure with normal ejection fraction (>=50%), as confirmed by echocardiogram within 2 years of initial admission. The researchers identified 1,141 HISDN-treated patients (949 black, 634 nonblack) and 15,565 patients not treated with HISDN (5,004 black, 6,500 nonblack). People of unidentified race were excluded from the analysis. The investigators compared 1-year mortality rates by racial subgroups, between those who were treated with HISDN and those who were not. One-year mortality rate was significantly different at 15.9% in the black group treated with HISDN and 20.8% in the black group not treated with HISDN (P = .0004). Also, HISDN provided a significant survival benefit for nonblack patients (P < .0001). “Despite the retrospective nature of this study, there is definitely evidence here to suggest that for this group of medications that is already indicated for systolic heart failure, there is reason to consider it and study it for diastolic heart failure,” Dr. Mazurek said.
