ERS: Weekly Chemotherapy Schedule 'Slightly Superior' to Standard Regimen For Non-Small-Cell Lung Cancer

GLASGOW, SCOTLAND -- September 15, 2004 -- A dose-adjusted weekly chemotherapy schedule using carboplatin and paclitaxel appears to be slightly more effective than the standard regime used for treatment of non-small-cell lung cancer (NSCLC).

Combination therapy using two platinum agents is currently the standard of care for first-line treatment of NSCLC, said lead investigator Christian Schumann, MD, Department of Internal Medicine II, University of Ulm, Ulm, Germany.

However, the preliminary results of a multicenter trial presented here September 6th at the European Respiratory Society 14th Annual Congress did not show clear superiority of one schedule over the other.

The 867 chemo-naïve patients enrolled in the study had advanced NSCLC and were randomized either to a weekly schedule of carboplatin at an area-under-the-curve (AUC) of 2 and paclitaxel 100 mg/m2 or to a standard schedule of carboplatin at an AUC6 and paclitaxel 200 mg/m2 once every 3 weeks.

Preliminary analysis of patients with sufficient follow-up to determine response to treatment included 767 participants, which was 97.6% of all patients. Of these, 223 had cancers in stage IIIb and 613 were in stage IV.

The results were mixed. The weekly schedule had slightly better objective response rates, but survival and time to progression were somewhat better with the standard regimen.

Complete response was achieved in 2.4% of patients on the weekly schedule and in 1.9% on the standard schedule. Partial response occurred in 33.9% of patients in the experimental arm and in 29.8% of the standard therapy arm. Median overall survival was 8.6 months for the weekly group and 9.3 months for the standard group. The time to progression was 6.7 months for the weekly schedule group and 7.7 months for the standard regime group.

There were no significant differences in either hematologic or non-hematologic toxicities between the two dosing schedules.
