ERS: Once-daily Ciclesonide and Twice-Daily Fluticasone Show Comparable Results in Adults With Asthma

GLASGOW, SCOTLAND -- September 7, 2004 -- Once-daily treatment with ciclesonide (Alvesco, Altana) is as effective as twice-daily fluticasone propionate (Flonase, GlaxoSmithKline) in adults with asthma.

In a study presented here September 6th at the European Respiratory Society 14th Annual Meeting, Dr. Roland Buhl and colleagues, University of Mainz, Mainz, Germany, said the convenience of once-daily administration of ciclesonide could enhance compliance relative to other drugs, and therefore could improve treatment outcomes for these patients.

In Dr. Buhl’s study, 230 patients were treated with ciclesonide 160 mcg/day and 221 with fluticasone 88 mcg BID for 12 weeks. Patients had baseline forced expiratory volume in 1 second (FEV1) of 50% to 90% of predicted values, but their symptoms were reversible with rescue medications.

Overall, improvements in lung function parameters were comparable between the two groups. FEV1 were similar in the ciclesonide and the fluticasone groups (+506 mL vs. +536 mL); forced volume capacity (FVC) increased slightly more in the ciclesonide group (531 mL vs. 523 mL), peak expiratory flow (PEF) increased more in the fluticasone group compared with the ciclesonide group (36 mL/min vs. 29 mL/min; P = NS between treatments).

The treatment changes for FEV/1, FVC and PEF in both groups were statistically significant compared with baseline (P <.0001). Similarly, asthma symptom scores were improved over baseline in both groups. However, there were no statistically significant differences between the two drugs on any measurements. Subjects using fluticasone required slightly fewer rescue medications than those given ciclesonide.

In conclusion, Dr. Buhl said, once-daily evening administration of ciclesonide 160 mcg/day is as effective as fluticasone propionate 88 mcg BID in improving lung function, reducing asthma scores and reducing the need for rescue medications in adolescents and adults with asthma.
