| | | ![]() Combined Systemic Chemotherapy, Hepatic Intra-Arterial Drug-Eluting Beads Effective for Unresectable Hepatic Malignancies: Presented at ESMO-GI By Jenny Powers BARCELONA, Spain -- July 1, 2009 -- Hepatic arterial infusion using a drug-eluting bead device offers effective therapy for the treatment of unresectable hepatic malignancies and is well tolerated, according to a study presented here at the 11th World Gastrointestinal Cancer Congress of the European Society for Medical Oncology (ESMO-GI). Robert Martin MD, University of Louisville School of Medicine, Louisville, Kentucky, and colleagues evaluated the safety and efficacy of combined systemic chemotherapy and precision hepatic arterial irinotecan or doxorubicin therapy to treat unresectable hepatic malignancies. Results were presented here on June 25. The primary endpoints of the study were safety, tolerance, tumour response rates (modified Response Evaluation Criteria in Solid Tumors) and overall survival (OS). This open-label, multicentre study included 310 patients of which 56 patients have been given 83 hepatic arterial drug-eluting bead treatments with irinotecan (54% of patients) and doxorubicin (46% of patients) in addition to concurrent chemotherapy. Systemic chemotherapy included irinotecan (18%), oxaliplatin (13%), cisplatin (10%), cetuximab (10%), and others. A total of 48 (85%) of the patients received the full planned number and dose of bead treatment in combination with systemic chemotherapy. The overall response rates were 70% at 6 months and 75% at 12 months. No deaths occurred within 1 month of treatment. Grade 1 to 3 adverse events (AEs) occurred in 30% of patients with the most common being nausea, vomiting, and pain. There was no evidence that AEs were related to the bead chemotherapy. The investigators concluded that hepatic arterial infusion with irinotecan or doxorubicin using a drug-eluting bead device was a safe and effective treatment for unresectable hepatic tumours. They said that additional trials are warranted to optimise usage of this therapy.
