| | | ![]() AAAAI: Sphenoid Sinusitis Recurrence Associated with Previous Sinus Surgery By Ed Susman DENVER, CO -- March 9, 2003 -- Doctors report that patients who have had surgery for sphenoid sinusitis are more likely to have recurrence than are patients not treated surgically for the disease. The condition, however, "can be successfully treated with medical management," said S. Bobo Tanner, MD, assistant professor of medicine at Vanderbilt University, Nashville, Tennessee, United States, speaking here March 9th at the 60th Annual Meeting of the American Academy of Allergy, Asthma and Immunology. Previous research by Dr. Tanner's team found that of 202 patients who underwent medical management of sinusitis that involved the sphenoid sinus, about 59% responded to up to 12 weeks of antibiotic therapy. To determine the recurrence rate of sphenoid sinusitis Dr. Tanner's and colleagues reviewed baseline and treatment computed tomography scans from 537 patients with sphenoid sinusitis. The patients ranged in age from 6 to 81 years with an average age of 46. About 55 percent of the cohort studied consisted of women. Of the 120 patients for whom antibiotic treatment was successful, Dr. Tanner said the condition recurred in 54.2% of patients from 1 to 26 months later. The average time to recurrence was about 8 months, he said. Length of treatment required to resolve the original sinusitis infection did not appear to be a risk factor for recurrence, Dr. Tanner reported. Similarly, patient age, sex and history of allergic rhinitis were not predictive of recurrence. However, 39% of patients who recurred had previous sinus surgery, compared with 8% recurrence among patients who did not have previous sinus surgery. "There is nearly a five-fold increased incidence of recurrence in these patients," Dr. Tanner said in his oral presentation. He said the study shows that recurrence is possible even after successful medical treatment of sinusitis involving the sphenoids with clearance of the disease confirmed by computed tomography scan.
