| | | ![]() DG DISPATCH - RSNA: Harmonic Power Doppler Enhances Patterns Of Hepatocellular Lesions By Lara Pullen Special to DG News CHICAGO, IL -- December 6, 1999 -- Differential diagnosis of small, nodular, hepatic lesions remains difficult. A recent study suggests that evaluation of the enhancement pattern of harmonic power Doppler imaging is a valuable, noninvasive means to investigate tumor vascularity.
Dr. Carlo Bartolozzi, of the University of Pisa in Italy, presented the results of this study at the 85th Annual Meeting of the Radiological Society of North America, in Chicago, IL. Sixty-one histologically-proven, liver tumors of hepatocellular origin (in 47 patients) were included in the pilot clinical study. Pathologic diagnosis included hepatocellular carcinoma (n = 37), macroregenerative nodule (n = 16), focal nodular hyperplasia (n = 6), and hepatocellular adenoma (n = 2). Examination was technically successful in 50 of 61 lesions (82 percent). The investigators encountered three distinct patterns of enhancement -- hypervascular, hypovascular and avascular. There was a 96 percent agreement when comparing Doppler ultrasound with tissue-core percutaneous biopsy and spiral computed tomography in the differentiation of hypervascular versus hypovascular lesions. Dr. Bartolozzi concluded that, despite the considerable overlap between benign and malignant tumors, this technique has the potential to help characterize hepatocellular lesions.
