PARIS, FRANCE -- July 5, 1999 -- Depletion of lining macrophages has taken place after clodronate liposomes (CLO-LIPO) administration, according to P. Barrera of department of rheumatology, University Hospital Nijmegen, Utrecht, Amsterdam.

Rheumatoid arthritis is a macrophage disease. Intraarticular administration of CLO-LIPO results in local depletion of phagocytic synovial lining cells and has prophylactic and therapeutic effects in mice models of arthritis.

This underscores the role of synovial macrophages in both early and chronic phases of arthritis and encourages further studies in humans.

Study results show that the intraarticular administration of CLO-LIPO is safe and effective on the lining macrophages in patients with rheumatoid arthritis.

The study evaluated 10 patients (eight women, two men) with rheumatoid arthrits (RA). The mean age was 63 years (48 to 78 years). They required a total knee replacement and had no known susceptibility to infections or bleeding. In the control group, there were three women and three men ranging in age from 53 to 73 years (median 63 years).

Knee biopsies were obtained percutaneously seven days before CLO-LIPO administration. The dose of CLO-LIPO was 159 +/- 36 mg. A second biopsy was obtained during surgery one week later. CLO-LIPO administration was well tolerated and no advers events had been observed.

Haematoxyle staining and immunohistology showed depletion of lining macrophages. The results show that administration of CLO-LIPO is safe. The therapeutic value of this procedure deserves further investigation, the researcher said.
